The Solution

This is the checklist of common errors found on the forms. A copy of this checklist was sent out with each set of forms. Since the supervisors now have to complete this checklist, it is hoped that the forms will be checked more carefully and the number of incomplete or incorrect forms reaching the office manager will decrease.

This is the memo sent out to the supervisors informing them of the new procedure involving the checklist.
