Executive Summary
Burke Engineering, Incorporated, recently discovered a problem in its employees' paperwork that led to quality defects in its operations. The problem arose when employees either filled out their initial employment forms incorrectly, or simply did not fully complete the forms. It was also discovered that the supervisors in charge of handling these forms were not detecting these errors properly. As a result, Burke's Office Manager was forced to delay her normal duties in an attempt to find the correct information. This not only led to a general slow down of normal business operations handled by the Office Manager, but the employees' paychecks were withheld until the correct information was found. A simple solution to the problem was implemented. The Office Manager attached a checklist that explained the proper steps needed to complete the forms. The job supervisor, and then the field supervisor, would monitor the forms and the checklist to make sure they were done correctly.
